The Dutch Safety Board has investigated an incident that occurred at Amsterdam Airport Schiphol on February 10, 2010. Several conditions contributed to a takeoff of a Boeing 737 from the taxiway. One of the recommendations of the Board has been a collaborative risk identification. For that purpose, MovingDot gathered all stakeholders, including the airport, ATC, pilots, towers and de-icers. Together, using BowTie methodology, a highly visual method for risk and consequence identification,  the most important risks in the taxi operation have been identified and structured.
